Originally established as a three-bed PUD (public utility district) in 1956, South Peninsula Hospital is a non-profit medical facility. With nine emergency rooms, three birthing center beds, eighteen acute care beds, an attached long-term care facility, and a plethora of ancillary clinics and doctor's offices, it has expanded over the course of six decades into a cutting-edge critical access hospital. The hospital is the product of a special collaboration between the City of Homer, SPH, Inc. (the non-profit organization that provides the healthcare), and the Kenai Peninsula Borough (which provides capital investments and service-area tax support for the facility).

A board of directors consisting of nine members oversees the hospital. Public recommendations on capital expenditures using tax dollars and service scope are made by an elected Service Area Board. The hospital is the biggest employer in the area, employing over 500 locals, and its payroll alone brings in over $20,000,000 a year for the local economy. The economic health of the southern peninsula is greatly impacted by this as well as service agreements, the purchase of materials, and leasing. Grants from the South Peninsula Hospital Foundation, Inc. and the South Peninsula Hospital Auxiliary provide some funding for ancillary projects.

The only public psychiatric hospital in the state of Alaska is called Alaska Psychiatric Institute (API). Any person in Alaska who needs to be hospitalized for a psychiatric crisis can receive acute, inpatient mental health services from API. Most patients treated at this hospital have severe mental illnesses that have persisted for a long time.

Furthermore, for those who have been determined by the court to be incompetent to stand trial, API is presently the only facility in the state offering competency restoration treatment.

1962 saw the Institute's opening. As part of the Alaska Mental Health Enabling Act, which provided funding of $6.5 million for its construction, it was put into service in 1956.

In 2019, those receiving care at the hospital self-reported that 41% of them were Caucasian, 38% were Alaska Native, 8% were African American, and 13% were "other." In 2019, 40% of adults ages 18 and older who self-reported as female and 60% as male were treated by API. In2019, just eight (8) teenagers between the ages of 13 and 17 received treatment before the treatment facility closed. The adolescent unit reopened in the spring of 2021 and is presently accepting 10 adolescents at a time, when it is at capacity.

The institute can accommodate up to 80 beds in addition to its normal 72 beds. There have been worries that this won't be sufficient to satisfy demand.

Three units comprise the beds: secure forensic, adolescent, and medium to long term.

In 2012, the Joint Commission acknowledged the institute for its exceptional performance, placing it in the top third of its nationwide rankings for psychiatric hospitals.
